By registering for and participating in the OQSL, I acknowledge and agree to the following:

I confirm that I am voluntarily participating and am physically able to do so. I understand that participation in softball and related activities involves inherent risks, including but not limited to physical injury, illness, permanent disability, or death, as well as loss or damage to personal property. I voluntarily assume all such risks, whether known or unknown.

In consideration of being permitted to participate, I, on behalf of myself, members of my family, my heirs, executors, administrators and assign,  I hereby waive, release, and discharge the OQSL, its directors, officers, employees, volunteers, sponsors, officials, the City of Ottawa, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to my participation, including travel to and from league activities, to the fullest extent permitted by law. I further agree to indemnify and hold harmless the OQSL and its representatives from any such claims, including those arising from negligence.

I confirm I have reviewed Rowan's Law: Concussion Awareness Resources.

I acknowledge that the OQSL is a 2SLGBTQIA+ inclusive and affirming space. I agree to conduct myself in a respectful and non-discriminatory manner toward all participants, including respecting individuals’ names, pronouns, identities, and expressions. I understand that harassment, hate speech, or discriminatory behaviour will not be tolerated and may result in removal from the league without refund.

In the event of an emergency, I authorize OQSL representatives to obtain medical treatment on my behalf if I am unable to do so, and I accept responsibility for any associated medical expenses.

I confirm that by submitting this registration electronically, I am providing my informed consent and that this acknowledgement constitutes the legal equivalent of my handwritten signature.
