My team and I agree to follow the rules and regulations outlined in the OQSL League Constitution and By-laws and will cooperate with the instructions of officials, the league executive, and tournament organizers. I certify that all information provided is accurate and consent to the sharing of the details within the Registration Form, Release, and Player Profile Form with the OQSL Executive and tournament organizers. I understand that I will receive a separate registration link and am responsible for ensuring all team players register individually and indicate the team they are playing for in the tournament. I understand that, I too, must complete the player registration form once received. I also confirm that I will submit the required payment ($1,000) by the specified deadline once notified by the tournament organizers.
